In the Layer Palette, click on Raster 1 and Flood Fill with
White.
Right click on the Fairy layer and Duplicate. Click on the
original Fairy Layer and Drag it down to just above Raster 1.
Resize this layer by 120%. (Image> Resize or Shft-S) In the
layer palette, turn down the Opacity of this layer to 56% (or
more if you want a lighter look). Using your Move tool,
position this layer behind the fairy so that the bottom edges of
the second sets of wings line up.

Click on your Eraser tool. Use size 15. Erase the feet that
are sticking out at the bottom below the fairy. There is no
need to be precise about this.

Now click on Effects> Distortion Effects> Curlicues. Use
these settings.

Right Click on your Feathers Layer and Duplicate. Click on
your Original Feathers layer. Turn the Opacity of this Layer
down to 56% (or whatever you turned the fairy layer down to).
Resize by 110%. (Shft-S) Use your Move Tool to drag this layer
up, behind and slightly above the other feathers.
Click on Effects>Distortion Effects> Curlicues. Use the same
settings except Increase the number of Rows and Columns to 13.
Click on your Text Layer and add a Drop Shadow. (Effect> 3D
Effects> Drop Shadow). Use these settings or your own.
Click on your Copy of Fairy Layer and add a Drop Shadow using
the same settings or your own. And you are done!! If you feel
like your name isn't showing up enough, you can lower the
Opacity of the Copy of Feathers layer slightly (to 90% maybe?)
Right click on any layer. Merge> Merge All (Flatten) Crop.
And save as a gif or a jpg.
